i have myrealname@yahoo, which i mostly use for ecommerce. i have a pseud@yahoo for internet companies i'm not doing money-business with and who i don't want o have my real name. i have a gmail account because it's neat and i got it early, so i got a good name. i don't use it for much though. i have a gmail account to which i forward all the email that goes to both my vanity-domains (the gaiman-related one and the one i host my other stuff on), because i got sick of dealing with the 1,000+ spams those accounts get every day (because the gaiman-account got so much press through his blog) and gmail deals better with spam than what i had set up. i have my work account which i only use for work stuff, and i can't access it from home. and i have my main account, myrealfirstname@mit which is where i do almost all my personal email, and which has reasonable spam-catching through MIT. i love that both because i've had it the longest of my current emails, and also because i got myfirstname as a username which makes me happy.
my first email was on a VAX server at my high school in 1991, but it is sadly defunct. i also had email at the BBS i used in high school, but that's also dead now.
